OBTAIN
A REPEAT PRESCRIPTION?
If you take regular
medication your doctor may give you a repeat prescription without the need for
you to be seen each time in the surgery. With our computerised repeat prescription,
you will receive a tear-off slip with each prescription issued which lists all
the items you have been prescribed for repeat use. This will normally be for one
month’s supply. At least three days before you are due to run out of medication,
return this slip to the surgery indicating which items you require - please tick
only those items you need. This will normally be ready for collection 48 hours
later.
Patients who are on regular stable medication and who would normally
use Lloyds Pharmacy are now able to ring the chemist direct to order their medication.
This means only one trip to the chemist to collect the medication or have it delivered
if appropriate. Please contact Lloyds Pharmacy to sign up for this new service.
Patients on stable repeat medication can also opt for the “Batch Prescribing Scheme” which allows the patient to
have an annual supply of prescriptions which are taken to the chemist of your choice without the need to attend
the surgery every month.
Please note that we do not accept repeat requests over the telephone but there are other
ways in which you may order a repeat prescription: by calling into the surgery
and putting the slip in at the reception desk, by posting it to the surgery, by
fax on 0191 454 6787 or, alternatively, you can now order your repeat medication
online - please ask at reception to register for this service. If you
supply a stamped addressed envelope, we will either post the prescription to you
or, alternatively, to the chemist of your choice.

A SICK NOTE?
A sick note completed and signed
by your doctor is only required when a period of continuous absence from work
due to illness is of seven days duration or longer. For absences of up to three
days no certificate is required. Between three and six days' absence due to illness
is covered by obtaining a self-certificate obtainable from your employer or the
surgery reception.
